TaliWood Cabinets, LLC sells premium kitchen cabinets and bathroom vanities through authorized dealers, builders, and showrooms. This application starts our review of your firm as an independent manufacturer's representative. Complete every field and submit. We review your application before any agreement is prepared. Fields marked with an asterisk are required.

Insurance
What to ask your insurer for
Ask your insurance company to send us a Certificate of Insurance that names TaliWood Cabinets, LLC as additional insured on your General Liability policy. Upload that certificate with this application (below).

Commission payment and system access
Confidential. We pay commissions by ACH direct deposit and use this only to set up payment and your Shopify access.

Certification, authorization, and signature
By submitting this application, on behalf of the firm named in Section 1, I certify and agree that:
- Accuracy. All information in this application and its attachments is true, complete, and accurate to the best of my knowledge, including the banking and ACH details in Section 8. The account named in Section 8 belongs to the legal entity in Section 1 and is authorized to receive commission payments by direct deposit. I will promptly notify TaliWood of any material change, including any change to the banking details.
- Authority to bind. I am authorized to sign on behalf of the legal entity named in Section 1 and to bind it. I am not signing on behalf of a trade name or DBA alone.
- Reference and verification authorization. I authorize TaliWood Cabinets, LLC to contact and verify the references listed above, to speak with the manufacturers and customers we work with, and to conduct reasonable business verification of the firm, including a business credit or background check. I release those references and TaliWood from any liability for good-faith disclosures made in connection with this review.
- Confidentiality. I understand that during evaluation and any resulting relationship we may receive confidential TaliWood information, including pricing, dealer lists, and program terms, and I agree to keep it confidential and use it only to represent TaliWood.
- No agreement yet. I understand this application is part of TaliWood's review and does not create a representative relationship or any obligation until a written agreement is signed by both parties.
